Most women with PCOS will conceive. The question is rarely "if" but "with which step on the ladder." This guide walks the full sequence: lifestyle, insulin-sensitising support, ovulation induction, IUI, and IVF, with the clinical decision criteria for moving from one rung to the next. How PCOS Affects Fertility (Quick Recap)

PCOS blocks fertility primarily through anovulation, the technical term for a cycle in which no egg is released. The ovaries can be full of small follicles (the "polycystic" appearance on ultrasound) but none mature and ovulate reliably. That, not the cysts themselves, is the conception barrier.

Egg quality in PCOS is generally normal at any given age. The issue is getting the egg out reliably and at the right time. That framing matters because most PCOS fertility treatment options exist to restore ovulation, not to fix the eggs themselves.

"What is the first-line treatment for PCOS infertility?" The answer in current international guidelines is letrozole, an oral aromatase inhibitor, supported by lifestyle foundations and insulin sensitisers where appropriate. We'll work up to letrozole through the earlier rungs.

Step 1: Lifestyle and Medical Optimisation (the Foundation)

I want to be clear about this step, because the framing online is often hostile. Lifestyle is not "fix yourself first" gatekeeping. It's the foundation that makes every subsequent treatment work better, and for many women with PCOS and an elevated BMI, it restores ovulation on its own.

The evidence is consistent on a few points. A modest weight change of 5 to 10% of body weight restores spontaneous ovulation in a meaningful proportion of women with PCOS and elevated BMI. Step 2: Inositol and Metformin (Insulin-Sensitising)

Once the lifestyle foundation is in place, insulin-sensitising support is often the next conversation. Two options dominate.

Myo-inositol, often combined with D-chiro-inositol in a 40:1 ratio, is dosed at approximately 2 g twice daily. It improves cycle regularity and ovulation in PCOS and is generally well tolerated. [src]

Metformin is a generic prescription drug that improves ovulation, particularly in women with BMI above 25, demonstrated insulin resistance, or impaired glucose tolerance. It can be used alongside letrozole when oral induction begins. Gastrointestinal side effects are common in the first weeks and usually settle with slow titration.

Both are adjuncts, not stand-alone "fertility drugs." Neither will reliably trigger ovulation in a non-ovulatory PCOS cycle on its own.

Step 3: Letrozole (First-Line Ovulation Induction)

Letrozole for PCOS is the first-line drug for ovulation induction in current guidelines from the 2023 International Evidence-Based PCOS Guideline, ASRM, the Endocrine Society, and ESHRE.

The evidence shift came from the PPCOS-II trial published in the New England Journal of Medicine in 2014. Letrozole produced higher ovulation rates and a meaningfully higher live birth rate than clomiphene in women with PCOS. [src] That single trial moved practice.

A typical regimen starts at 2.5 mg daily on cycle days 2 to 6 and titrates up to 5 mg or 7.5 mg over subsequent cycles if ovulation isn't achieved. Cycle monitoring with follicle tracking scans is standard where access permits, especially at higher doses.

Risks are modest. Multiple pregnancy (mostly twins) sits around 3 to 5% per cycle. Hot flushes are the most common side effect. Letrozole does not thin the endometrial lining the way clomiphene can.

Step 4: Clomiphene Citrate (Second-Line, Still Useful)

Clomiphene was the first-line drug for decades and is still widely prescribed, particularly where letrozole access is limited. NHS prescribing patterns in some regions still default to clomiphene because letrozole remains off-label for ovulation induction in the UK (used off-label routinely by specialists but not always at GP level).

The trade-offs versus letrozole: a higher rate of thin endometrial lining (which can reduce implantation), more multiples, and lower live birth across most age bands per PPCOS-II.

Clomiphene still has a role, particularly when cost, prescribing constraints, or patient preference favour it.

Step 5: Gonadotropin Injections and Ovarian Drilling

When oral agents fail across multiple cycles, two further options sit on the ladder before IVF.

Gonadotropin injections (FSH, sometimes with LH) are injectable hormones that drive follicle development directly. They're effective but require close monitoring, cost more than oral agents, and carry a higher risk of multiple pregnancy and ovarian hyperstimulation. Delivery is through a fertility clinic, not GP-led.

Laparoscopic ovarian drilling is a targeted surgical procedure (small electrocautery or laser punctures of the ovarian surface) that restores ovulation in a subset of women with clomiphene- or letrozole-resistant PCOS. It's used selectively now, more often where IVF access is restricted or specifically indicated, and isn't a first-line option for most patients. [src]

Step 6: IUI vs IVF in PCOS

IVF with PCOS sits at the top of the ladder, but it's not always the next step after letrozole.

IUI (intrauterine insemination) is typically combined with ovulation induction. It's helpful when there's an additional mild factor (mild male factor, cervical, anatomical) on top of the ovulation issue. As a stand-alone for pure anovulatory PCOS, IUI adds less value than people expect because the limiting factor was getting an egg out, not where the sperm sits.

IVF is indicated when first- and second-line induction has failed, when additional factors (tubal disease, severe male factor) coexist, when age is pressing, or when time matters. "When does PCOS need IVF" is usually answered by the failure pattern: 3 to 6 ovulatory cycles of letrozole without conception, or earlier escalation for women in their late 30s.

IVF in PCOS comes with one important caveat: ovarian hyperstimulation syndrome (OHSS) risk is higher because PCOS ovaries respond strongly to stimulation. Modern antagonist protocols, GnRH agonist triggers instead of hCG, and freeze-all strategies have made IVF substantially safer in PCOS than it was a decade ago. Special Considerations: Lean PCOS, Older Reproductive Age, GD Risk

The ladder above is the standard pathway, but three sub-groups need adjustment.

Over 35 with PCOS: skip prolonged trials of lifestyle alone. Move to ovulation induction promptly, and set a defined number of letrozole cycles before escalating. Time is itself a factor at this point.

Gestational diabetes risk: PCOS roughly doubles the risk of gestational diabetes in the resulting pregnancy. How Long Does Each Step Take?

This is the honest timeline I give patients so they can plan their lives, not just their cycles.

  • Lifestyle plus inositol: 3 to 6 months before judging whether ovulation has restored.
  • Letrozole ovulation induction: typically 3 to 6 ovulatory cycles before escalating.
  • IUI with induction: 3 to 4 cycles before considering IVF.
  • IVF: cycle by cycle, individualised by response.

A typical full pathway runs 6 to 18 months, not 6 weeks. Knowing this in advance removes a lot of the "is it working yet" anxiety in the early months.

UK

UK / NHS: GP first for diagnosis and baseline bloods. Referral to NHS fertility clinic for ovulation induction. Letrozole is off-label in the UK for ovulation induction but commonly prescribed by specialists; some areas still default to clomiphene at primary care level. NHS IVF funding is governed by NICE NG257 but commissioned locally, with age caps and BMI thresholds that vary by Integrated Care Board.

US

US: Faster access via OB-GYN or directly to a reproductive endocrinologist (REI). Letrozole prescribed routinely for PCOS ovulation induction. Insurance coverage for IUI and IVF varies substantially by state and employer; about 20 states have some IVF mandate. Out-of-pocket IVF cycles commonly run $15,000 to $25,000.

How does PCOS affect miscarriage risk?+

Can I conceive naturally with PCOS without any medication?+
